How to complete your team packet
Information for returning your Team’s paperwork.
 
Codes to use for this year’s paperwork:

            League #  = 09, if playing in Mid NJ Youth Soccer Association
            DC # = 62
            Club# = Cranford = 9053                           
                                                      
Make sure you use the latest forms.  In addition to getting these forms on this website, you can also download the forms from the NJ Youth Soccer website.
 
For the players,  submit the following papers for each team in the SAME ORDER AS THE ROSTER (paper clip each of these five forms separately) DO NOT USE STAPLES:
 
1.     PAPER CLIP NJYS Membership Form (facing front) and copy of Birth Certificate (facing out on the back) together.  Make sure the Birth Certificate clearly shows the player’s age.
 
2.     Player Passes  (Absolutely NO corrections of any kind, no cross outs, no white outs, etc).  Make sure that the player pass number -01 is included on the Team Roster line 1 and that player pass number -02 is on line 2 of the Team Roster and so on.  If you don’t have a player for -03 lets say, leave that line blank on the team roster.
 
3.   Original versions of the Medical Release Forms are required (no copies).  The Medical Release Form can either be signed by an Attorney or notarized by a Notary Public.  If notarized, the document must include a raised seal. 
 
4.     The Player & Parent Sportsmanship Pledge form that MUST be signed by both the player and parent.  If you need copies of the “Kids Come First” video please let me know.
 
 
For the coach:
 
1.     You must read the NJ Youth Soccer Coach Handbook (that is included in this email).  This handbook gets into more specifics of how to go about completing the paperwork.
 
2.     Copy of coaching license.
 
3.     Completed NJYS Membership Form.  Most coaches are Volunteers so the NJ Volunteer Coach form (attached in this email) is to be completed.  If a coach is paid in any way (as a trainer for example), they MUST have a Non-Volunteer Coach Membership Form completed by the NJ State office.
 
4.     Coach’s Pass  (Absolutely NO corrections of any kind).
 
5.     Coaches to sign the Sportsmanship Pledge.
 
Please make enough copies for all the players on the team of the "NJ Youth Soccer Membership Form", and the "Medical Release Form".  The S.A.G.E. forms can be found on the Mid NJ website at http://www.mnjysa.org/ under the SAGE link.
 
When filling out the New Jersey Youth Soccer  Membership Form, use DC# 62, League # 09, Club # Use the four digit club number for your club from the attached chart and your Team # is a four digit number which can be found in the upper right hand corner of your Team Roster.
