7. BIRTHDATE:  Enter Birthdate in format, MM/DD/YYYY

8. TEAM:  Enter Club & team name, e.g. Cranford Cougars, NOT just Cougars

9. AGE GROUP:  Enter Team Age as U12, U13, etc.  DO NOT enter G or B or Girls or Boys after it
